Good Faith Estimate Notice

Effective Date: July 4, 2026

You have the right to receive a Good Faith Estimate explaining how much your healthcare services may cost.

Under the No Surprises Act, healthcare providers are required to provide uninsured and self-pay clients with a Good Faith Estimate of expected charges for services.

You are considered uninsured or self-pay if you do not have health insurance, choose not to use insurance, are not using insurance benefits for services, or are receiving services from an out-of-network provider and paying privately.

Your Right to a Good Faith Estimate

You have the right to receive a Good Faith Estimate for the total expected cost of non-emergency healthcare services, including counseling services.

A Good Faith Estimate will include expected charges for services that are reasonably expected to be provided. This may include intake sessions, individual counseling sessions, assessments, or other services discussed as part of your care.

When You Will Receive a Good Faith Estimate

You may receive a Good Faith Estimate when you schedule services or upon request.

You may request a Good Faith Estimate before scheduling services. If you schedule services at least three business days in advance, you may be entitled to receive a written Good Faith Estimate within the required timeframe.

Estimated Counseling Fees

Fees may vary based on service type, session length, and clinical need. The following are estimated self-pay rates:

Initial Intake Session: $150 per session
Individual Counseling Session: $95 per session
Extended Session: $125 per session
Consultation Call: $0
No-Show or Late Cancellation Fee: $65

The actual number of sessions needed will vary depending on your concerns, goals, progress, availability, and treatment preferences. Because therapy is individualized, it is not always possible to know in advance exactly how many sessions will be needed.

This estimate is not a contract and does not require you to receive services from Rebuild Within Counseling and Wellness PLLC.

Changes to Your Estimate

Your Good Faith Estimate may change if your treatment needs change, your session frequency changes, additional services are requested, or you choose to continue therapy longer than initially estimated.

You may request an updated Good Faith Estimate at any time.

Disputing a Bill

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill through the federal patient-provider dispute resolution process.

You are encouraged to save a copy of your Good Faith Estimate for your records.

Insurance and Out-of-Network Services

Rebuild Within Counseling and Wellness PLLC may be an out-of-network provider. If superbills are provided, they may be submitted by clients to their insurance company for possible reimbursement. Reimbursement is not guaranteed and depends on your insurance plan.

Clients are responsible for understanding their insurance benefits, out-of-network coverage, deductibles, reimbursement policies, and financial responsibility.
